The waiver deadline for Spring 2010 has passed.

However, you may submit a Waiver appeal* by following the directions below.  Appeals will be accepted until
March 19, 2010

*Submitting an appeal does not guarantee that it will be approved or that the charge will be removed from your account.

1.  Print out the Waiver Appeal Form
2.  Complete the form and attach a copy of your insurance card (front and back)
3.  Return the completed form to the Student Health Center*

*All appeals must be hand delivered to the Student Health Center.  Appeals that are mailed or faxed in will not be accepted.

                                                  

 

Need to know if your health insurance plan qualifies?

  • Any commercial health insurance plan that provides insurance coverage in the United States and is comparable to the Student Health Insurance Plan.  (i.e. Aetna, Blue Cross Blue Shield, Cigna)

  • Medicaid

  • Medicare Part A and B

  • Tricare (formerly known as CHAMPUS)

  • CHAMPVA

  • Department of Veterans' Affairs (must be eligible for medical treatment at VA Medical Center and have a valid ID card)

               **All registered students are welcome to use the Student Health Center regardless of their health insurance plan**
