Immunization Department Location:
The Department is housed on the upper level of the Student Health Center located on the corner of Mary Alexander Rd. and Cameron Blvd. Appointments are advised for administration of vaccines or consult with a Registered Nurse regarding vaccines, allergy shot administration, or travel.Mail Records to Us:
UNC CHARLOTTE STUDENT HEALTH CENTER - IMMUNIZATIONS DEPARTMENT - 9201 University City Blvd. Charlotte NC., 28223-0001      All records must be mailed or hand delivered.  Faxed copies will not be accepted.

Need a Copy of Your Immunization Records? Copies of Immunization Records may be obtained by filling out an Authorization of Release of Medical Information and contacting the Student Health Center Medical Records Department at 704-687-7192.

Unsigned copies of your immunization records may also be viewed and printed via Online Student Health.
